What is Direct
Primary Care?
The DPC model allows for a smaller panel size, transparency in cost and more time for personalized care.
Dr. Young and Dr. Koda started this practice after realizing that the traditional (“fee-for-service”) model was prohibitive in providing the type of care they believe patients deserve.
Direct Primary Care (DPC) is a healthcare model where patients pay physicians a flat, affordable monthly membership fee directly for comprehensive primary care services, bypassing traditional insurance billing and administrative hassles for these services.
Our doctors want you to feel heard, seen, and to leave the clinic knowing you are in good hands.
It’s important to note that while DPC covers a wide range of primary care services, it does not replace insurance. Patients are still encouraged to maintain a health plan that fits within their budget for hospitalizations, specialty care, and emergencies

Meet the team
Dr. Emi Koda
Dr. Koda has provided comprehensive care to families in a variety of clinical settings up and down the PNW for the last 15+ years (as well as Buffalo, NY!). Each setting has been rewarding but in recent years she found that the existing model of healthcare delivery could not allow her to provide care in the way she envisioned. She finds joy in really getting to know her patients, their background and what motivates them. She believes in shared decision making and meeting the patients where they are through each season of life.
She is interested in all facets of family medicine and appreciates a whole body approach to medicine.
Outside of doctoring she tries to keep up with her runner husband, two kids, two dogs and geriatric cat. They can often be found roaming Mt. Hood in the winter and getting out on bikes and trails in the Summer.
Dr. Brandon Young
Dr. Young believes the best way to take care of patients is to know them and understand their health care goals.
As a member of the LGBTQ+ community, he believes that it’s essential to offer a safe and comfortable space for all patients to receive care.
In addition to general primary care, he offers gender-affirming care, supports medical weight management, and ADHD testing and treatment.
Dr. Young completed all of his education and training locally in Portland, Oregon. He earned a bachelor of science in biochemistry at Portland State University. He earned a doctor of medicine at Oregon Health & Science University. From there, he completed his residency in internal medicine at Providence St. Vincent Medical Center.
Away from work, Dr. Young enjoys traveling with his husband. Before becoming a physician, he had a career in footwear design and worked with many Portland footwear companies.

No, we do not bill any insurance companies so that we can cut out the administrative burden and “red tape”

Yes. You will have the option to bill your insurance for labs and imaging but we will not be able to provide the exact cost. If you ‘self-pay’ we can often order at a discounted rate and would be able to provide you the estimate

We recommend that our members keep their insurance to cover unexpected larger medical expenses such as going to the emergency department, hospitalizations and seeing specialists
However, insurance is certainly not a requirement to be a member

Yes, we can place referrals to specialists and you will be able to use your insurance to see them (as long as they are in your network)

We anticipate the rate to be approximately $125-$175 per month per member (app $1800 per year, discounts may apply)

Yes, as of 2026 the monthly membership fee can go toward your annual deductible

Yes, as of 2026, you can now access your HSA to pay the monthly membership fee

No, we do not prescribe any chronic opioids, benzodiazepines or sleep aids. Your long term health is so important to us and we aim to treat based on current evidence and safety.
We do prescribe controlled substances to treat ADHD

Once we are up and running we will schedule a meet and greet to answer any (non-medical) questions and to see if we would mutually be a good fit

We will have standard office hours during the week. This can be via HIPAA compliant text, phone call, virtual platforms or in person.
If there is an emergency after hours we would need to direct you to urgent care or the emergency department if it is not something that can be safely addressed via phone.
We aim to be accessible by phone after hours but we also want to have a sustainable practice for many years to come!

It originated from the japanese crane “tsuru.” The crane symbolizes longevity and loyalty, two values that we hope to instill in our clinic.
You may have heard of the symbolism of a thousand cranes “senbazuru.” A thousand folded paper origami cranes are thought to provider healing to the ill.

There is a parking lot behind the building with free parking and also free street parking.

15-20 min prior in case you get lost or have trouble with parking. We have a strict 15-Minute Late Arrival Policy to ensure that you have the best experience with your provider. If you arrive more than 15 minutes into your visit, your appointment may be rescheduled or you may be asked to wait until there’s an available time with the doctor .

Please complete and sign all required forms before your first appointment. These include your medical history, demographic information, privacy policy, HIPAA acknowledgment, and patient agreement. You’ll receive these documents by email, and once submitted, they’ll be sent directly to our front office.

If your visit involves completing any medical paperwork (such as disability leave, FMLA, employer leave forms, or sports/medical clearances), please email those documents to patientsupport@XXXXX.com ahead of time.
