Direct Primary Care (DPC) is a healthcare model where patients pay physicians a flat, affordable monthly membership fee directly for comprehensive primary care services, bypassing traditional insurance billing and administrative hassles for these services.
Patients are still encouraged to maintain a health plan that fits within their budget for hospitalizations, specialty care, and emergencies

There is a parking lot behind the building with free parking and also free street parking.

15-20 min prior in case you get lost or have trouble with parking. We have a strict 15-Minute Late Arrival Policy to ensure that you have the best experience with your provider. If you arrive more than 15 minutes into your visit, your appointment may be rescheduled or you may be asked to wait until there’s an available time with the doctor .

Please complete and sign all required forms before your first appointment. These include your medical history, demographic information, privacy policy, HIPAA acknowledgment, and patient agreement. You’ll receive these documents by email, and once submitted, they’ll be sent directly to our front office.

If your visit involves completing any medical paperwork (such as disability leave, FMLA, employer leave forms, or sports/medical clearances), please email those documents to patientsupport@XXXXX.com ahead of time. This allows Dr. Koda or Dr. Young to review them and confirm they can be completed during your appointment.
